Please provide all the necessary documentation including:
Identification
Copies of U.S. Customs and Immigration Services (USCIS) documents showing legal status in the U.S. for any non-citizens.
Utility Bills
A copy of your most recent & active utility bills from each utility supplier (both gas and electric)
Income
Proof of ALL earned and unearned gross income received by ALL household members 18 years or older in the month prior to the month of your application. (If you apply in December, provide income received in November)
Paid Medical & Dental Expenses
Proof of medical/dental expenses you paid out during the month prior to the month of your appointment or application date. (If you apply in December, provide medical expenses paid in November)
Child support and alimony
Proof of any child support and/or alimony you paid out or received in the month prior to the month of your appointment. (If you apply in December, provide proof of Child Support and/or Alimony you paid or received in November)
Proof of Disability
Letter of decision from Social Security administration or a letter from a doctor indicating that he/she is permanently disabled or disabled for six months or longer. No diagnosis, please.
